I’ve always had trouble figuring out where to place links. If someone knows, please tell me which one of these is the right way.

I prefer #4. The link is located on the context of the pointer. Linking on the name or source is vague because you might link to them more than once and yet all links look similar. I hope you know that the text you use to hyperlink only helps the source you are linking to (strategy used for Google bombing)
